* It is very important to eat if you have taken insulin, as the insulin will lower your blood sugar regardless of whether you have eaten. If you take insulin without eating, you could have hypoglycemia. This is called an insulin reaction.

* There is an adjustment period while you learn how insulin affects you and how to time your mealtimes and exercise times with your insulin injections to keep your blood sugar level as even as possible.

* Keeping accurate records of your blood sugar levels and insulin dosages is crucial in helping your health care provider take care of your diabetes.

* Eating a consistent, healthy diet appropriate for your size and weight is essential in controlling your blood sugar level.

Type 2 diabetes: Depending on how elevated your blood sugar and glycosylated hemoglobin are at the time of your diagnosis, you may be given a chance to lower your blood sugar level without medication.

* The best way to do this is to lose weight if you are obese and begin an exercise program.

* This will generally be tried for 3-6 months, and then your blood sugar and glycosylated hemoglobin will be rechecked. If they remain high, you will be started on an oral medication, usually a sulfonylurea or biguanide (Metformin), to help control your blood sugar level.

* Even if you are on medication, it is still important to eat a healthy diet, lose weight if you are overweight, and engage in moderate physical activity as often as possible.

How Diabetes Affects Your Vision

The American Diabetes Foundation reports that more 23.6 million children and adults in the United States who have diabetes. Another 5.7 million people are undiagnosed and often unaware they suffer from diabetes.

Diabetes is a disease in which the body does not produce or properly use insulin. There are two types of diabetes; Type 1- is when the body fails to produce insulin, the hormone that allows cells to turn suage and starch to energy. Type 2- is when the body is resistant or fails to peoperly use the insulin, this is the most common form.

Symptoms & Signs

There generally are no early warning signs or pain associated with diabetic retinopathy. Characteristic signs of retinal changes may be seen during examination of the retina. Severe retinopathy may develop prior to noticeable vision loss, with late sudden onset of vision loss with hemorrhage or retinal detachment.

With the symptoms of proliferative retinopathy, if bleeding occurs at first, you will see a few specks of blood, or spots, "floating" in your vision. If spots occur, see your eye care professional as soon as possible. You may need treatment before more serious bleeding occurs. Hemorrhages tend to happen more than once, often during sleep.
Sometimes, without treatment, the spots clear, and you will see better. However, bleeding can reoccur and cause severely blurred vision. You need to be examined by your eye care professional at the first sign of blurred vision, before more bleeding occurs.

If left untreated, proliferative retinopathy can cause severe vision loss and even blindness. Also, the earlier you receive treatment, the more likely treatment will be effective

Cause & Risk of vision damage

Approximately 40 percent of all people with diabetes have at least mild diabetic retinopathy, which affects both Type 1 and Type 2 diabetics. Diabetic retinopathy is a major cause of blindness and tends to be particularly severe in Type-1 diabetes.

In general, the longer one has had diabetes, the greater one`s chances of developing diabetic retinopathy. Diabetic retinopathy generally takes at least eight years to develop in juvenile onset diabetes, but may be present at the time adult onset diabetes is first diagnosed. High blood pressure and high cholesterol in the blood are additional risk factors that your doctor will address.

Diagnosis
Because diabetic retinopathy can begin and get a foothold before it causes symptoms, all patients with diabetes should have an eye examination with pupils dilated at least once a year. Often your doctor will take photographs of your retinas. A dye, called fluorescein, may be injected in the blood stream to show further details of the circulation in the retina. This is called fluorescein angiography.

Diabetic retinopathy has four stages:

1. Mild Nonproliferative Retinopathy. At this earliest stage, micro aneurysms occur. They are small areas of balloon-like swelling in the retina`s tiny blood vessels.

2. Moderate Nonproliferative Retinopathy. As the disease progresses, some blood vessels that nourish the retina are blocked.

3. Severe Nonproliferative Retinopathy. Many more blood vessels are blocked, depriving several areas of the retina with their blood supply. These areas of the retina send signals to the body to grow new blood vessels for nourishment.

4. Proliferative Retinopathy. At this advanced stage, the signals sent by the retina for nourishment trigger the growth of new blood vessels. This condition is called proliferative retinopathy. These new blood vessels are abnormal and fragile. They grow along the retina and along the surface of the clear, vitreous gel that fills the inside of the eye. By themselves, these blood vessels do not cause symptoms or vision loss. However, they have thin, fragile walls. If they leak blood, severe vision loss and even blindness can result.

Proliferative Diabetic Retinopathy

Treatment

Proper treatment of diabetic retinopathy can cut the risk of vision loss by over half. Treatment before diabetic retinopathy causes severe loss is much more effective than later in the disease. For this reason, early diagnosis is critical in order to prevent visual loss and blindness. Control of diabetes and blood pressure is important; intensive control of blood glucose can delay onset and slow progression of retinopathy.

There are treatments for diabetic retinopathy, which usually include laser surgery or ‘vitrectomy’. It involves removing the cloudy vitreous and replacing it with a salt solution. Because the vitreous is mostly water, you will notice no change between the salt solution and the normal vitreous. Although laser surgery and vitrectomy are very successful, they do not cure diabetic retinopathy. Once you have proliferative retinopathy, you will always be at risk for new bleeding. This means you may need treatment more than once to protect your sight.

Prevention

There are ways to prevent or lessen the risk of eye damage caused by diabetes.
The Diabetes Control and Complications Trial proved that careful control of blood glucose levels might prevent the development of diabetic retinopathy.
Regular eye examinations are especially important for children who have had diabetes for 5 years or longer, for adults at the time of diagnosis, for those who have difficulty controlling the level of sugar in their blood, and for diabetic women who are considering becoming pregnant. All of these people are at increased risk for diabetes-related eye problems. To protect your vision get a comprehensive dilated eye exam at least once a year and remember that damage to the vision can occur without symptoms.

Future Risk

In a recent Washington Post article entitled ‘Diabetic Eye Disease Rates Soaring’, the story quotes a study showing the rates for cases of diabetic retinopathy soaring from 5.5 million to nearly 16 million cases by the year 2050.The study gave the following breakdown;

-The number of cases of vision threatening diabetic retinopathy will increase from 1.2 million to 3.4 million.

-Among Americans 65 and older, the number of cases of diabetic retinopathy will rise from 2.5 million to 9.9 million, and the number of cases of vision threatening diabetic retinopathy will increase from 500,000 to 1.9 million.

-Cataract cases among whites and blacks age 40 or older with diabetes will increase 235 percent. Cataract cases among people age 75 and older with diabetes will increase 637 percent for black women and 677 percent for black men. Glaucoma cases among Hispanics age 65 and older with diabetes will increase 12-fold.

Diabetic coma is a medical condition in which, a patient suffering from diabetes loses his/her consciousness as a result of imbalance in blood sugar level. A patient suffering from very low blood sugar (known as severe hypoglycemia) or very high blood sugar (known as severe hyperglycemia) can get into diabetic coma.

Diabetic coma can be a result of few medical conditions such as:

  • Severe hypoglycemia as mentioned above, a medical condition in which patient suffering from low blood sugar level becomes unconscious. This condition is reversible and can be brought under control by following treatment regime strictly and eating/drinking something (carbohydrates) immediately after the patient experiences the symptoms of diabetic coma.
  • If patient is experiencing dehydration and has severe hyperglycemia, it can lead to diabetic coma (also known as Nonketotic Hyperosmolar coma)
  • If patient is undergoing physical fatigue and experiencing dehydration along with a severe hyperglycemia, it can lead to diabetic coma (this condition is also called Advanced Diabetic Ketoacidosis or DKA in which the patient’s condition starts worsening and after about 35 hours the patient may get into coma if not treated)

What happens in diabetic coma?
As mentioned above, diabetic coma can be a result of either hyperglycemia or hypoglycemia in combination with dehydration and fatigue. In all the cases, a patient loses consciousness (coma) due to the effect of hyperglycemia/hypoglycemia on brain cells. Glucose is one of the most important elements for human body that helps in keeping metabolism in proper condition. If glucose level in blood drops beyond normal (hypoglycemia) or is higher than maximum glucose level our body can handle (hyperglycemia), it hinders normal metabolism of body and brain cell start malfunctioning. This condition results in variance in osmotic pressure on the cells causing dehydration and other conditions like electrolyte imbalance. This condition can be reversed if patient is treated before it is too late.

Symptoms of diabetic coma:

  • Patient with high blood sugar may experience dehydration condition, frequent vomiting and/or urination, queasiness, dryness of mouth and shortness in breathing (deep breathing). The patient may also experience increase in thirst.
  • Patient with low blood sugar may experience physical fatigue, increase in hunger, increase in sweating, sudden shiver etc.
  • Patient may experience pain in abdominal area

Treatments available:

  • If severe hypoglycemia leads to diabetic coma, the treatment used for this condition involves raising of blood sugar with the help of medical glucose that can be injected into patient’s body.
  • KDA or Advanced Diabetic Ketoacidosis and Nonketotic Hyperosmolar coma treatment involves constant supply of saline and medical solution which can help in filling up the loss of the body fluid during vomiting in earlier stage of KDA in combination with appropriate dosages of insulin. The treatment may vary based on the patient’s medical condition and severity of the diabetic coma.
  • Your physician will decide the course of treatment that needs to be followed based on your condition.

What to do to help patient with diabetic coma?

  • If the patient is conscious and experiencing symptoms of diabetic coma, immediately inform doctor. If patient has lost consciousness then immediately take him/her to a doctor. (Delay in treatment may lead to Acidosis or KDA (Advanced Diabetic Ketoacidosis). Extreme delay in medical help can also lead to death)
  • If possible try to find out sugar level in blood (either ask the patient if he/she is still conscious or use glucose meter). If the patient is suffering from hypoglycemia, then it will be dangerous to give him/her insulin in such case. Patient suffering from hypoglycemia should be given something to eat/drink which will help in restoring blood sugar level immediately. Be careful not to give anything to eat/drink to a patient suffering from hyperglycemia.
  • If you are confused about the patient’s condition and don’t know what to do to keep the patient in conscious state, please DO NOT GIVE PATIENT ANYTHING EXCEPT FOR WATER.
  • In case if cause of diabetic coma (hypoglycemia or hyperglycemia) is unknown, DO NOT GIVE INSULIN to the patient, it can be very dangerous.

Tips for people suffering from diabetes:

  • Consult your physician regularly and follow treatment regime strictly prescribed by your physician.
  • Keep your blood glucose level under check.
  • Regular exercise can help you keep glucose level under check (excess exercise can create problems for people suffering from hypoglycemia)
  • If possible, wear a medical bracelet/necklace or carry a medical card in your wallet that identifies you as a diabetic patient, this can be of great help.
  • Try to teach people around you what they should do in case there is an emergency.

It is common that women who are not diagnosed with diabetes previously have shown high blood glucose level (hyperglycemia) during pregnancy. This type of diabetes is known as gestational diabetes. It is estimated to affect about 3-8% of pregnant women worldwide. Usually, gestational diabetes is seen in the second half of pregnancy; however, blood glucose levels become normal after the baby is born. In case the symptoms of diabetes persist after giving birth, then the condition is called chronic diabetes, which can be either Type 1 diabetes or Type 2 diabetes.

Gestational diabetes is caused due to the hormonal changes that take place in all women during pregnancy. It is similar to Type 2 diabetes in which the body cells become resistant to insulin hormone. Insulin induces the body cells to take up glucose from the blood for using it as a source of energy.

Due to the problems of body’s cells that respond to insulin, glucose remains in the blood and hence results in hyperglycemia. Symptoms of gestational diabetes are polydipsia (increased thirst), polyuria (increased urination frequency) and fatigue.

Diet

Gestational diabetes doesn’t pose an immediate threat to the health of the pregnant woman. However, it should be controlled as it increases the risks of certain complications such as premature labor, secretion of excess amniotic fluid and preeclampsia (hypertension and presence of protein in urine). In addition, untreated diabetes can cause jaundice, breathing problems and hypoglycemia in the baby.

Gestational diabetes can be managed well with proper diet, regular exercise and routine monitoring of blood glucose levels. Blood glucose levels less than 105 mg per dL (fasting) and less than 120 mg per dL (2 hours after meal) are considered to be normal. Let’s look into some of the useful diets that help in controlling high blood glucose levels during pregnancy.

Carbohydrates are the main source of glucose. Eat foods that contain adequate amount of carbohydrates and maintain the same amount of carbohydrates that you consume in each meal. Some of the healthy carbohydrate diets are pasta, whole-grain bread, bagel, rice, oatmeal and pancake. Vegetables contain fewer amounts of carbohydrates (about 5 grams per serving); accordingly one can plan the carbohydrate intake while including vegetables in the diet plan.

Along with carbohydrate intake, diet should be supplemented with protein-rich foods and sufficient amount of fats. During pregnancy an extra amount of proteins (about 12 grams) is required for the proper development of the fetus. Overall, the calorie intake should be such that about 45-60% should come from carbohydrates, 15-25% from proteins and 25-30% from fats.

Add fiber-rich foods to the diet such as fruits, fresh vegetables and bran cereals. Fiber slows down the digestion of carbohydrates that helps in controlling the blood sugar levels. It also helps in regular bowel movement.

It is advisable to consume four servings of dairy products and calcium-rich products such as milk, nuts, vegetables and other calcium fortified foods. Also, one should include foods rich in iron (fish, eggs and vegetables), vitamin A (pumpkin, carrot and spinach), vitamin C (tomato, oranges and strawberries) and folic acid (beans, lentils and almonds) in the daily diet, so as to get adequate nourishment during the pregnancy.

Drinking eight glasses of water per day is recommended to maintain a proper fluid balance in the body. Always include ample amount of fruits and vegetables in the diet plan. Consuming three meals and two to three snacks a day at regular intervals is beneficial. Try to eat meals at the same time everyday and don’t skip meals or snacks. This helps in maintaining the blood sugar level within the normal range. While consuming packaged food, one should always check the nutritional levels.

Avoid foods and drinks that have high sugar content such as corn syrup, molasses, jams and jellies. Alcoholic beverages should be strictly avoided as alcohol consumption can lead to premature birth and low weight of the baby at birth.

Women who are obese, older than 25 years at the time of pregnancy and who have a family history of diabetes should be more careful while planning for pregnancy. It is advisable to seek advice from a dietitian for proper planning of the meals. Consult your physician in case the blood sugar levels remain high even after following recommended diets and exercise. In such cases, the physician may recommend insulin injections.

Refined sugar, sweets and other sweet food or drinks, fats like butter and hydrogenated vegetable oil should be avoided, along with many processed foods. Alcohol and smoking should be avoided completely, if possible, though an occasional drink is not out of the question.

Salt should be reduced along with red meat and other foods that are full of saturated fats. Whole grains and fresh fruits should be added along with raw vegetables, sprouts, fish and other foods rich in antioxidants. Trans-fats and saturated fats should be replaced with monounsaturated fats like peanut, canola and olive oil.

Other foods that help are cinnamon sticks eaten naturally or in tea and fenugreek seeds soaked in water. The Indian gooseberry and margosa or neem extracts (azadirachta indica - a subtropical tree) have all proved useful in the treatment of type 2 diabetes. Garlic has been tested and found to be useful in limited studies. It offers antioxidant properties and micro-circulatory effects and could help reduce blood glucose, increase insulin secretion, and slow its degradation.

Exercise

Walking, any sport, jogging and swimming are particularly helpful. Specific yoga postures help in stretching and relaxing the body along with helping the mind to handle any stress.
